A Case Report of Hypercalcemic Nephropathy Manifesting Renal Salt Wasting and Vasopressin Resistance

Abstract

|
We describe here a case with Na-wasting and vasopressin resistance probably due to. hyper-calce mia, This 50 years old, male patient has sufferred from diabetes insipidus for 26 years and finally developed a lung cancer which metastasized to the bones. Hypercalcemia (8) pmEg/L) and by percalciuria, (39.6mEg/day) were found and associated with the Na-wasting (around 1, 000mEE/da y) and vasopressin resistance. A kidney necropsy showed mesangial proliferation, periglomerular fib rosin, metastatic nephrocalcinosis and intimal sclerosis of the small arteries and was compatible with hypercalcemic nephropathy. Literatures were reviewed and the authors concluded that salt wasting and vasopressin resistance were probably due to the effect of hypercalcemia.
